Summary: LAUKOA is seeking two full-time Licensed Practical Nurse (LPN) Case Managers to provide non-clinical medical case-management and administrative support to the Louisiana Army National Guard State Surgeon Office in Pineville, Louisiana. The Case Managers will track and manage the medical status of assigned Soldiers through military systems of record and support medical readiness requirements. Contractor personnel shall not perform inherently governmental functions or exercise Government decision authority.

Job Description

• Review privacy rights with each Service Member and obtain the appropriate signed release of information, including DD Form 2870 or the required VA-specific authorization.
• Review MEDPROS, MEDCHART, MODS, medical readiness portals, TEAMS, and other applicable DoD systems before medical readiness events to identify non-deployable Soldiers and required services.
• Track and manage the medical status of all assigned Soldiers through military systems of record and review, action, and update every open case at least once every 30 calendar days.
• Check and action all referrals from MEDCHART and other DoD systems daily by scheduling follow-up appointments with Army medical providers, requesting additional medical documentation, and completing provider-directed actions.
• Schedule profile examinations and evaluations at medical readiness events.
• Correspond with medical readiness officers and Soldiers to obtain information required for case-management files and to support Soldier attendance and submission of follow-up documentation.
• Manage each Soldier case from identification of the condition until the Soldier is medically ready and returned to duty or administratively separated.
• Document Service Member interviews, interactions, and case updates in MEDCHART within 24 hours of the interaction.
• Scan, index, and enter medical and dental documentation into the appropriate systems of record within five business days after receipt or a medical readiness event, and stamp documents “MEDPROS Posted” as required.
• Educate Soldiers about their medical readiness responsibilities and resources available to improve medical and dental readiness.
  • Review, track, and update health-assessment laboratory results; contact Service Members and provide education on lifestyle modifications.
  • Update MODS and other military systems of record for immunizations, pregnancy designations, eye examinations, and medical warning tags.
• Run MEDPROS reports before medical readiness events to compare rosters and identify medical needs, and run post-event reports to verify completion of scheduled services and current medical status.
• Bring Soldiers with permanent PULHES numerical designators of three or four to the attention of the State Surgeon Office and track applicable Integrated Disability Evaluation System referrals.
• Prepare medical documentation for Line of Duty investigations in coordination with Health Service Support.
• Prepare individual vouchers in the Automated Voucher System when a Soldier cannot complete Individual Medical Readiness requirements at a medical readiness event.
• Comply with Army Regulations 40-501 and 40-502, applicable DoD and combatant-command standards, Army National Guard guidance, and all HIPAA requirements.
• Attend and provide case-management and administrative support at all required medical readiness and Soldier Readiness Processing events at Camp Beauregard and at other locations when travel is approved.
• Remain current on all guidelines and regulations affecting case management and medical records and provide newly required system-generated reports as DoD systems change.

Qualifications

• Must be a Licensed Practical Nurse (LPN) with a current license in good standing and maintain all required credentials and qualifying employment background checks throughout contract performance.
• Must complete and maintain annual HIPAA certification, DoD cyber-awareness and information-assurance training, installation-access requirements, and systems authorization before accessing Service Member records.
• Must be fully trained and otherwise qualified to perform case-management services and safeguard protected health information, personally identifiable information, and Government systems.
• Experience with Army National Guard medical readiness, MEDCHART, MEDPROS, MODS, eCase, Health Readiness Records, and Microsoft Office is preferred. Must be available to work full-time on-site in Pineville, Louisiana, Tuesday through Friday from 0700 to 1730, and support approved readiness-event travel when required.
